Form 5 - Responsible declaration for the execution of works

If you wish to carry out typical work (tiling, flooring, bathroom, kitchen, etc... or similar) the corresponding form is for a responsible declaration, Urbm005, and you must attach the basic documentation (Photocopy of DNI, IBI receipt, site plan which is provided by the website of the cadastre, a detailed budget and proof of payment of municipal taxes (Rate plus ICIO), and if applicable, it must be complemented with the documents described on the front of the form. All documentation can be submitted on the Municipal website.

Relevant information to complete the tax document:

Select CONCEPT: Fee for processing Urban Authorizations
Select SUBCONCEPT: Responsible declaration for works that do not involve new construction or expansion and changes of use.
Once the RATE has been made, you must repeat the same previous procedure to make the ICIO payment
